Fees & Payment


FEE FOR SERVICES
Fee for services are charged by the session. Standard sessions last 45 minutes.
Customary Fees for standard sessions are charged at the rate of $125.00 for initial assessment interviews, and $115.00 for succeeding treatment/consultation sessions. Sessions that exceed the 45- minute standard sessions are pro-rated at the base charge.

Usual and customary fees are charged for all services. If no third party (insurance benefits) payments are available to the client or applicant, it will be completely at the discretion of the therapist to consider a reasonably lower non-customary fee, if requested due to a particular hardship.

If third party (insurance benefits) payments are available, forms will be completed and submitted as promptly as possible. However, please be advised that all fee payments are basically the direct responsibility of the client or applicant.

To avoid being bill for services not covered by your insurance. prior to service it is best to call the 800 number on the back of your insurance card and verify that your that your are covered for the services your are seeking from the provider prior to your initial office visit.  Check to be sure that prior authorization is or is not required. 


When third party payments have been or are being paid directly to the therapist, adjustments and/or reimbursement credits will/may be made accordingly.

All co-payments or fees are expected to be paid in full at the end of each session unless other special arrangements have been made in advance of the session. Feel free to discuss any matters of concern with your therapist at the beginning of the session.

If paying by check, please save time by making it out in advance.

UNCANCELLED APPOINTMENTS:
Canceled appointments require at least 24-hour notice.
Uncanceled and No-Show appointments are charged as regular appointments

Medical Insurance


PRIVATE INSURANCE COMPANIES (PacificCare, ValueOptions, Blue Cross/Shield, Aetna, Cigna, etc)

The courtesy of billing your insurance will be extended to you, upon request. However, in order to provide this service, you must furnish our office with complete insurance information, have written confirmation of your coverage and necessary authorization, if necessary, and complete all insurance forms.
Deductibles and co-payments must be paid up front, prior to beginning of each session.

The amount your insurance company will pay can only be estimated, since each insurance company has their own specific limitations and exclusions. When co-payment is not clearly specified or figured on a percentage basis, then payment of 50% of standard fees will apply, until otherwise notified.

When payment is not received from your insurance company within 45 days of billing, the unpaid balance will become the patient’s responsibility.

All account balances are expected to be cleared within 45 days or less of the last contact.

Ultimately, it is the patient’s responsibility for the costs of all services, whether or not the insurance company honors claims that are submitted. Therefore, you should be sure of your coverage, eligibility, and if the company will cover services rendered by your therapist.  You should also check to be sure if prior authorization for services are required.  Often this is mandatory for coverage. Services not covered or authorized by your insurance company will be billed directly to the patient.
